Issue of New Passports / Re-issue of passports/ Jumbo Passports
Document required
As per instructions in general guidelines, printout of duly filled online application form available at https://embassy.passportindia.gov.in/, should be submitted at BLS. All particulars in the application should tally with those in the current passport. Passport will be retained during the period of the service
Two white background photographs. (Please refer to the photo specification mentioned in general guidelines)
Current Passport in original along with photocopies of the first, last, address page (if separate page), any other endorsement pages and visa page of the passport and additional booklets, if any.
A copy of ID card / residence card.
Note:
If the passport has expired for more than three year, Prior approval is required from Consular Officer.
Applicant should be ordinarily resident of Oman and the applicant should come personally.
Prior approval of Embassy of India is required for any major changes in the details different from current passport. The applicants can approach concerned officer at the Embassy directly or by appointments taken through BLS Intl.
Change of name:
In case of married women, change in surname Change of Name Form duly filled in and signed by the applicant should be submitted along with the application as below:
Following is prescribed for addition /deletion/change of spouse name in the Passport as well as change of name and surname consequent upon marriage/ divorce etc. :
(I) For addition of spouse name in passport:
(a) Marriage Certificate.
or
(b) Joint Photo declaration signed by both husband & wife.
(II) For deletion of spouse name:
(a) Divorce order/ decree
(III) For Change of spouse name:
(a) Divorce order/decree or death certificate of first spouse and
(b) Re-marriage certificate or Joint Photo declaration signed by both husband & wife
(IV) For change of surname by women applicant following marriage/ divorce:
(a) Marriage Certificate or Joint Photo declaration by both husband & wife
(b) Divorce order/ decree (if name/surname change is based on divorce)
Spouse name can be included by writing the spouse name clearly on the application. However, it is advisable to submit the spouse passport copy or name id so that correct spelling is incorporated on the passport.
In other cases, the change in names has first to be advertised in a daily newspaper (as per format of affidavit attested in Embassy), both in Oman and in India at the area of your residence.
The applicant should first visit the Consular officer for verification and affidavit to be attested with the following documents before newspaper advertisement. The following documents are required for verification: -
Newspaper advertisement form
At least two public/school documents in the desired name to ascertain that the applicant has actually changed his/her name.
Original passport and its copy
Oman residence card in original and its copy.
Applicant should visit the Consular OFFICER After 30 days from the date of publication of advertisement in the newspaper. The following documents to be submitted along with online application form available at https://embassy.passportindia.gov.in/.
Clippings of two original newspapers or the Gazette notification of the concerned state
At least two public/school documents in the desired name to ascertain that the applicant has actually changed his name.
Two white background photographs. (Please refer to the photo specification mentioned in general guidelines)
Current Passport in original along with photocopies of first (personal particulars), last (address) pages and any other endorsement pages and visa page of the passport and additional booklets, if any.
A copy of ID card / residence card also to be enclosed.
Change of address:
In order to change the address any document evidence with changed address like original Election Commission ID card, water, electricity or telephone bill in applicant's name, nationalized bank passbook with 6 months bank statement (with permanent address mentioned in the passbook attested and signed by bank official along with applicant photograph) and Aadhaar card issued by UIDAI, Government of India is accepted. For minors, parent's passport copy will suffice to change address. All documents should be in original for verification and copy should be attached along with the application.
