Requirements for Life Certificate: -

Self-Declaration Form A (2copies)

The person should come to the Embassy with his Original Passport and its copy

Photocopy of valid ID card / residence card original and copy

Two recent passport size photographs

The person should come to the Embassy with his Original Passport and its copy

Photocopy of valid ID card / residence card original and copy

Two recent passport size photographs

Fee                     :   OMR 1.400

Submission time : 0900 hrs. to 1230 hrs.

Collection time    : 1430 hrs. to 1600 hrs.

Note: 

Applicant may contact Indian Embassy number 98282270 for facilitation if they find it difficult to visit Embassy for applying for Life Certificate.
