Surrender/ renunciation Certificate

1. Applicant need to visit Embassy to check if there is any penalty, if he has travelled in Indian Passport after acquiring other nationality.
2. As per instructions in general guidelines a duly filled online application form available at https://embassy.passportindia.gov.in/,should be submitted at BLS. All particulars in the application should tally with those in the current passport.
3. Two white background photographs. (Please refer to the photo specification mentioned in general guidelines)
4. Current Passport in original with photocopy of the first, last, and address page and any other endorsement pages and visa page with additional booklets, if any, also to be produced.
5. A copy of ID card / residence card also to be enclosed.
6. Acquired nationality passport copy.
